Medicare and Medigap help in San Francisco, California

San Francisco is a compact city and county, but many residents see doctors in the East Bay, the Peninsula or Marin, which matters when an Advantage plan's service area and network are county based. City and public-sector retirees may also have retiree health benefits to coordinate. We help San Franciscans weigh these choices by phone or video.

Care across Bay Area counties

If your specialists are in another Bay Area county, check that each one is in a plan's network, or consider whether Original Medicare with a supplement fits better. Network participation can change each year.

Retiree health benefits

If you have retiree coverage from a city, state or private employer, changing to an individual Medicare plan may end that coverage permanently. Ask your benefits administrator before enrolling elsewhere.

Medicare Supplement (Medigap) in San Francisco

California's birthday rule lets current Medigap policyholders switch to equal or lesser benefits each year in a window starting on their birthday, without health questions. New enrollees get the federal six-month Medigap Open Enrollment Period once they are 65 and in Part B.

Medicare Advantage and Part D in San Francisco County

Medicare Advantage and Part D plans are offered by county service area, so options in San Francisco can differ from neighboring counties. The 2027 Annual Enrollment Period runs October 15 through December 7, 2026; your Initial Enrollment Period around age 65 is separate.

Help with costs

If your income and resources are limited, Medi-Cal, Medicare Savings Programs or Extra Help with Part D may apply. Eligibility is decided by the state or Social Security, not by an agent.

Questions to ask before you choose

  • Is my San Francisco address in San Francisco County for plan purposes?
  • Are my doctors and pharmacy in the plan's network?
  • Does Medigap with Part D or a Medicare Advantage plan fit me better?
  • Do I qualify for any program that lowers my costs?

Frequently asked questions

  • Can I keep my retiree plan and add Medigap? It depends on how your retiree plan coordinates with Medicare. Many retiree plans already fill gaps, so a supplement may be unnecessary. Review the plan documents first.
  • Does RGI Group have an office in San Francisco? No. Our only office is in Austin, Texas. San Francisco residents are served by phone or video by an agent licensed in California.
